Breaches and what happens next
Once per period, on the card of the work it is about, and only for the person accountable.
One alert per period
Indicators are re-evaluated regularly, but a breach is raised only once per period. Re-evaluating updates the result but never raises a second alert.
When the next period starts, if it is still over the line, it breaches again.
If the measurement could not be taken at all — there was no data — no breach is raised. "We could not measure" must not wake anybody up.
Where a breach appears
On the Pulse card of the engagement it is about, with both numbers side by side: what was measured, and what the limit was.
That card is shown only to the indicator's owner. Showing it to everybody turns accountability into spectacle.
If the owner did not ask for a Pulse card on this indicator, no card appears however loudly it breaches.
The task it raises names the worst case
A breach can raise a task, and that task names the worst engagement of the period rather than being a general alert.
If the breach happens at a level with no specific engagement to name, no task is raised — a task attached to "?" belongs to nobody.
Deleting an indicator
Deleting an indicator takes its recorded measurements and breaches with it — those are readings of this definition.
But the tasks raised from breaches stay. Somebody may be doing one right now, and deleting a measurement must not delete somebody's work.
